Key Responsibilities:
• Facilitate navigation through mental health and addiction services
• Provide information and brief support via multiple engagement methods
• Screen contacts for risk and prioritize client needs
• Coordinate referrals to community partners for appropriate care
• Maintain an updated directory of local services
Requirements:
• Two-year diploma in Human Services or related field
• Minimum one year experience in mental health and addiction services
• Knowledge of community resources and screening tools
• Effective communication skills in English; French is preferred
• Acceptable Police Vulnerable Sector Check upon hire
