Your leadership will facilitate collaboration with key healthcare professionals and enhance behavioral care plans within the designated long-term care home. This role requires a comprehensive understanding of geriatric assessments and the application of best practices in behavior management.
Key Responsibilities:
• Provide expert clinical support and guidance to LTC staff




• Coach staff on behavior care plans and frameworks
• Conduct clinical assessments for responsive behaviors
• Collaborate with various health care professionals
• Document care plans and maintain accurate records
Requirements:
• Current registration as a Registered Practical Nurse in Ontario
• Minimum one year experience in long-term care
• Training in GPA, U-FIRST, and P.I.E.C.E.S.
• CPR/First Aid certification
• Demonstrated ability to mentor and lead teams
