Position Summary

The Post Majority Care Case Manager supports young adults transitioning from the child welfare system to Independent adulthood. The Case Manager works directly with youth aged 18-25 who were formerly in care, assisting them to achieve stability in housing, education, employment, mental health, and life skills.

This position ensures youth receive coordinated supports consistent with Post Majority policies, Indigenous Services Canada (ISC) guidelines, and Sandy Lake First Nation Cultural and community values. The Case Manager acts as an advocate, mentor, and service coordinator to promote long-term well-being and independence.

Key Responsibilities

Case Management

- Provide individualized case management services for youth enrolled in the Post Majority Care program.
- Conduct Intake assessments and develop comprehensive individualized plans with each youth.
- Maintain regular contact with youth through meetings, check-ins, and crisis support as required.
- Monitor progress towards goals related to housing, education, employment, health, and life skills.

Youth Support & Advocacy

- Advocate for youth in accessing services such housing, education, employment training, healthcare, disability services, and counselling.
- Assist youth in navigating systems including education, social services, justice and healthcare.
- Support youth in developing essential life skills including budgets, cooking, conflict resolution, and time management.
- Provide crisis intervention and emotional support when necessary.
- Assist youth in applying for eligible compensation settlements.

Program Coordination

- The PMC Case Manager should be an expert in all regional and community support services as a critical part of the PMC role is to liaise with services and build in maximum supports into a client's case plan.
- Coordinate services with community partners, including:
- All Sandy Lake First Nation Programs and Services
- Tikinagan, Animikii and other CFS Agencies




- Indigenous and non-indigenous service providers
- Housing organizations
- Educational Institutions
- Employment programs
- Mental Health and addiction services
- Prevention services
- DSO, or CLDS
- Facilitate referrals and ensure youth receive appropriate supports.
- Work collaboratively with Mamow Prevention and Band Rep programs to define and set out key areas of collaboration.

Documentation & Reporting

- Maintain accurate case files and documentation in accordance with Mamow policies and privacy legislation.
- Prepare case notes, service plans, and progress reports.
- Keep an ongoing case lists of all clients you serve.
- Track program outcomes and service utilization for reporting.
- Participate in audits, program evaluations, and data collection activities

Cultural & Community Engagement

- Incorporate Sandy Lake cultural values and traditions into youth programming where appropriate.
- Support youth in strengthening connections to culture, community, and identity.
- Participate in cultural programming, ceremonies, or community events when appropriate.
- Facilitate activities to connect youth to a supportive peer support network.

Collaboration & Professional Practice

- Work collaboratively with internal staff including program coordinators, finance staff, administrative coordinator and other case workers at the Winnipeg Office.
- Participate in case conferences, supervision meetings, and team planning.
- Maintain professional boundaries and adhere to agency policies and ethical standards.
- Work alongside the Director and Coordinator to provide Input into the development of policies, procedures for the post majority care program.

Finance





- Record all program expenditures into QuickBooks with detailed notes.
- Ensure that all expenditures are included in a youths individualized plan.

QUALIFICATIONS

Education

- Diploma or degree in Social Work, Child and Youth Care, Indigenous Studies, Psychology, or a related field
- Equivalent education and experience may be considered.

Experience

- Experience working with youth transitioning from care, at-risk youth, or Indigenous communities preferred.
- Knowledge of child welfare systems and Post Majority Care programs considered an asset.
- Experience with case management, advocacy, and crisis intervention.

- Strong relationship-building and interpersonal skills
- Ability to work independently while managing multiple cases
- Knowledge of trauma-informed and culturally responsive approaches
- Excellent organizational and documentation skills
- Conflict resolution and problem-solving skills
- Ability to maintain confidentiality and professional ethics.

Other Requirements

- Valid driver's license and access to a reliable vehicle(if required for community travel)
- Clear Criminal Record Check and Vulnerable Section Check
- Ability to work flexible hours when necessary to support youth needs
- Knowledge of Sandy Lake culture, language and traditions.
- Other duties as assigned by immediate supervisor

Working Conditions

- Combination of office work, community outreach, and meetings with youth.
- May involve travel within the community or region.
- Position may involve supporting youth experiencing crisis situations,
