Make a difference in the lives of individuals living with dementia at SaskAbilties as a part-time Dementia Care Facilitator. Focus on enhancing quality of life through meaningful activities.

This permanent position of .5 FTE requires you to coordinate individual and group life enrichment activities both at home and in the community. Youll build trusting relationships with care partners to provide person-centered support, working collaboratively alongside team members to meet clients needs effectively.

Key Responsibilities:
• Coordinate life enrichment activities for individuals and groups
• Facilitate supportive living services for health aging
• Build trusting relationships with care partners
• Work collaboratively with team members for client satisfaction
• Promote person-centered planning and support

Requirements:
• Rehabilitation Worker Diploma or equivalent
• Valid Class 5 Driver’s Licence and reliable vehicle
• First Aid/CPR/AED certification, or willing to obtain
• Experience with dementia or disabilities
• Robust organizational and interpersonal skills
