Support mental health clients as a Transitional Case Manager with the Canadian Mental Health Association in Strathroy, Ontario. This full time position involves engaging individuals with mental health concerns, offering information and brief support alongside temporary case management. Your role will include assessments and collaborating with various community partners to ensure effective support.
