Reports To The Child Well-Being Coordinator reports to and is under the direct supervision of the Health Director.
Job Overview The Child Well-Being Coordinator provides community-based support to children, youth, and families in Marten Falls First Nation. Working as part of the community health team, the position focuses on prevention, early support, family strengthening, healthy child development, and connecting children and families with appropriate health, social, educational, cultural, and community services.
The coordinator builds trusting relationships with children and families, helps identify needs and strengths, supports referrals and service coordination, and assists with programs that promote safety, wellness, culture, belonging, and positive family relationships. Services are delivered in a respectful, confidential, culturally safe, and family-centred manner.
Responsibilities and Duties The duties include but are not limited to:
· Provide practical, strengths-based support to children, youth, parents, caregivers, and families.
· Build supportive relationships with children and families and help identify their needs, strengths, goals, and available supports.
· Support prevention and early-intervention activities that promote child and youth wellness, healthy development, family stability, and community connection.
· Provide outreach and follow-up with children and families, including approved home or community-based visits.
· Support children and families during periods of increased stress or crisis and connect them with appropriate professional or emergency services.
· Plan and support child, youth, parenting, family, recreation, cultural, and wellness programs in collaboration with the Nursing Station and other community programs.
· Promote opportunities for children and families to connect with language, land, Elders, cultural teachings, and community activities.
· Connect children and families with health, mental wellness, education, social, cultural, Jordan’s Principle,
and other appropriate services.
· Help families navigate referrals, appointments, service plans, and available supports, and follow up to reduce barriers to accessing services.
· Work collaboratively with Nursing Station staff, community programs, schools, child and family service providers, Elders, leadership, and other partners.
· Maintain accurate and confidential records of contacts, referrals, follow-up, activities, and program participation.
· Prepare activity summaries, statistics, workplans, and reports required by the Health Director and program funders.
· Identify service gaps and emerging child and family wellness needs and bring them to the attention of the Health Director.
· Maintain professional boundaries and protect the privacy and confidentiality of children and families.
· Participate in team meetings, training, case conferences, community events, and program planning as required.
Qualifications
· Diploma or degree in Social Service Work, Child and Youth Care, Indigenous Wellness, Community Health, Social Work, or a related field. A combination of relevant education, training, and community experience may also be considered.
· Experience working with children, youth, and families.
· Knowledge of child and family wellness, prevention, trauma-informed practice, and culturally safe service delivery.
· Knowledge of community health and social service systems, including referrals and service coordination.
· Experience supporting child, youth, family, wellness, recreation, or cultural programming is an asset.
· Current First Aid and CPR certification, or willingness to obtain.
· A clear and current Vulnerable Sector Check.
· Valid driver’s licence is an asset.
· Ability to speak Oji-Cree/Anishinaabemowin is an asset.

· Ability to recognize when additional professional, clinical, protective, or emergency support is needed and follow appropriate referral and reporting procedures.
· Knowledge of First Nations community realities, family systems, child and youth wellness, and prevention-focused approaches.
· Ability to plan and support group programs, workshops, family activities, and community events.
· Flexibility, reliability, sound judgment, and the ability to respond calmly to changing community needs.
Working Conditions
· Primary work location is Marten Falls First Nation, with work connected to the community health team and Nursing Station-funded programming.
· The role includes community outreach and may include approved home or community-based visits.
· Participation in community events, training, team meetings, and program activities may be required.
· The position requires flexibility and the ability to respond appropriately to changing child and family wellness needs.
