Consults with, and informs Physicians regarding client status, modifying nursing care plans accordingly. Coordinates service and provides nursing care, for assigned clients, at the level of a Nurse Practitioner. Performs comprehensive medical/nursing assessments and treatments, including (but not limited to) physical and cognitive evaluations; diagnosis and implementation of treatment; ordering and interpretation of diagnostic investigations (i.e. laboratory and diagnostic imaging); Authorizes medications in accordance with the Prescribing and Distribution guidelines set out by CRNA.

Provides preventive and therapeutic health teaching to clients and families, encourages independence and assists clients with adjustments to limitations. Functions in a consultative capacity to support and compliment the Home Care case managers and interdisciplinary team to work to their full scope of practice. As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan.

You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients.
