We’re looking for a Community Health Navigator and Coordinator to join our Community Health Services (CHS) team.
This role is ideal for someone who is people-focused, organized, compassionate, and comfortable navigating systems and connecting individuals to the right supports. You’ll work directly with clients, healthcare partners, community organizations, and our CHS team to help make healthcare and community services more accessible.
? Location: MNLC Finch office + community
? 35 hours/week
? $32/hour
? Community Health Services
?️Spanish language is required
