Hospice Intake Coordinator in Gold River, California

As a Hospice Intake Coordinator, you play an important role in helping patients and families access compassionate hospice care when they need it most. You’ll coordinate incoming hospice referrals, communicate with patients, families, referral sources, and our hospice sales team, and help ensure each patient receives timely, seamless, and compassionate support throughout the intake process.

As our Hospice Intake Coordinator, you will:

- Receive, log, and coordinate all incoming hospice referrals.
- Serve as the primary point of contact for patients, their families, referral sources, and external agencies.
- Provide clear, accurate information to patients/clients and referral sources to assist in accessing appropriate hospice services.
- Conduct complete intake screenings and ensure accurate data entry into the management information system.
- Collaborate with the hospice sales team and clinical staff to schedule patient visits and Hospice RN admissions.
- Prepare reports, respond to inquiries, and complete audits as required.
- Actively promote hospice services to prospective patients and referral partners.
- When permitted by law, receive verbal orders from physicians to initiate care/services.
- Maintain access to personal health information (“PHI”) in accordance with HIPAA and company policies.
- Actively participate as part of a high-performance team to drive change and deliver exceptional patient/client service.

A few must-haves for our Hospice Intake Coordinator:

- Preferred current Licensed Vocational Nurse (LVN) in the state of California.
- One (1) year of experience in intake coordination, hospice,



or home health office based experience.
- Computer proficiency, including the ability to learn company software.
- Strong knowledge of hospice philosophy and patient-centered care practices.
- Excellent communication, customer service, and organizational skills.
- Ability to work independently and collaboratively within a team.
- Demonstrated professionalism, attention to detail, and ability to prioritize multiple tasks in a fast-paced environment.
