Part-Time Transfer Coordinator - Toronto Animal Health Partners
Hourly wage: $26.00–$35.00/hour, based on experience.

Toronto Animal Health Partners (AHP), located in North York, is a state-of-the-art 24-hour emergency and specialty referral hospital serving dogs and cats across the Greater Toronto Area and beyond. Our hospital was purpose-built to provide advanced veterinary care in an environment designed around collaboration, innovation, and exceptional patient outcomes. AHP brings together experienced veterinarians, RVTs, and board-certified specialists across a variety of disciplines, including Emergency, Critical Care, Surgery, Neurology, Cardiology, Internal Medicine, Oncology, Anesthesiology, and more. Our facility features sophisticated medical infrastructure, including advanced diagnostic imaging, an in-house laboratory, a dedicated ICU, and a 3 Tesla MRI, the first of its kind in private veterinary practice in Canada.

About the Role
We are looking for a highly organized, communicative, and detail-oriented Transfer Coordinator to join our team in a fast-paced emergency and critical care environment. The Transfer Coordinator plays a key role in coordinating patient transfers, supporting communication between referring veterinary hospitals and our medical teams, assisting with client communication and financial coordination, and helping ensure patients move efficiently through the hospital.
This is a highly collaborative role that works closely with DVMs, RVTs, veterinary assistants, client service representatives, referring veterinarians, rescue organizations, and external referral hospitals. The ideal candidate is comfortable balancing multiple priorities, communicating confidently with both veterinary professionals and pet owners, and remaining calm and organized in a busy emergency environment.
What You'll Do
Transfer Coordination
- Receive and coordinate transfer requests from referring veterinary hospitals.
- Assess hospital capacity, including available staffing,
treatment space, equipment, and appropriate medical services, to determine whether a patient can be accepted.
- Coordinate with ER, Critical Care, and other hospital services to determine the most appropriate destination for each patient.
- Document potential and confirmed transfers accurately in ezyVet.
- Notify the appropriate teams of incoming transfers and ensure urgent or STAT cases are clearly communicated to the ER team.
- Monitor hospital capacity as patient volumes and anticipated admissions change throughout the day.
- Ensure all relevant medical records, diagnostic results, imaging, and other documentation are received from the referring clinic and attached to the patient's file.
Critical Care Client Communication & Support
- Attend morning ICU rounds and identify patients being managed by the Critical Care service.
- Confirm the daily patient list and relevant updates with the DVM and RVT teams.
- Provide appropriate client updates following rounds, based on information provided by the medical team.
- Maintain accurate and up-to-date Critical Care billing.
- Monitor patient accounts and communicate with the medical team when new estimates or deposits are required.
- Discuss financial updates with owners, obtain signed estimates, and collect required deposits.
- Assist with finalizing billing and completing client checkout for discharges and euthanasias.
- Track discharged Critical Care patients and complete appropriate follow-up communication within 48 hours.
- Monitor ezyVet for client memos, emails, and concerns requiring follow-up.
- Ensure updated medical records are sent to referring veterinarians when appropriate.
Emergency Service Support
- Review the daily ER patient list with the Back Vet and identify patients requiring billing or administrative support.
- Assist with billing for ER patients remaining in hospital under the ER service.
- Communicate with owners regarding updated estimates when patients require an extended stay or transfer to another service.
- Assist with obtaining appropriate financial authorization and deposits when required.
Blood Product Coordination
- Coordinate requests from referral hospitals requiring blood products.
- Work with TAHP and other referral hospitals to facilitate the purchase or acquisition of blood products.
- Ensure appropriate invoicing is completed for blood products purchased on behalf of other hospitals.
- Maintain accurate documentation related to blood product requests and purchases.
Rescue & Surrender Coordination
- Communicate with rescue organizations to identify appropriate organizations able to assume financial responsibility for surrendered patients.
- Work with the attending DVM to determine which rescue organization is appropriate for the patient.
- Coordinate the ownership-transfer process with the client and rescue organization.
- Ensure all required surrender and ownership-transfer documentation is completed.
- Update ownership and client information accurately in ezyVet.
- Apply the appropriate rescue discount to patient invoices following transfer of ownership.
Diagnostic Result Tracking
- Regularly monitor the ER/CC diagnostic tracking system for newly available results.
- Communicate straightforward negative results to clients when appropriate and in accordance with hospital protocols.
- Escalate positive, abnormal, or complex results to the appropriate DVM for review and follow-up.
- Ensure the medical team is notified of results requiring further action.
- Maintain accurate documentation of diagnostic results, including when and by whom results were communicated.
What We're Looking For
We are looking for someone who:
- RVT designation is preferred.
- Has excellent verbal and written communication skills.
- Is highly organized and able to manage multiple competing priorities.
- Remains calm, professional, and solution-focused in a fast-paced environment.
- Has strong attention to detail and takes pride in accurate documentation.
- Demonstrates sound judgment and knows when to escalate concerns to the appropriate medical team.
- Is comfortable working with medical records, diagnostic results, and confidential client information.
- Has experience with ezyVet and Excel, or demonstrates the ability to learn new systems quickly.
