Maple Villa is an independent 93-bed licensed long-term care home located in Burlington, Ontario. Maple Villa is fully accredited by Accreditation Canada and has been providing quality resident and family-focused care since 1971. We continually strive to make a difference, to provide Person and Family-centered care and provide services where safety is a priority.
Our interdisciplinary team are committed about the care for our residents, staff, families and volunteers and are consistently reflecting on practices and areas of improvement to provide exceptional, individualized, culturally competent care to our residents and maintain a robust and dedicated health care team.
We are opening a new Home in January 2027 that will support 256 residents in our community! The new Maple Villa is located on Palladium Way in north Burlington.
Position Summary
Under the direction of the Administrator, the Resident/Family Services Coordinator is responsible and accountable for providing a smooth transition for resident admission and continued placement. Ensures and provides support to residents and families before, during and following the Admission Process acting as a resident advocate. The incumbent will ensure that continuous quality improvement outcomes approved for the Home are met or exceeded, and demonstrate dedication to professional values and the mission, vision and values of the Home.

Qualifications
- Must have a current general certificate of registration with the Ontario College of Social Workers and Social Service Workers; or
- Must have a post-secondary diploma or degree in recreation and leisure studies, kinesiology, therapeutic recreation or other related field from a community college or university, and at least one year of experience in a health care setting as required under the Fixing Long Term Care Act, 2021, ss 79 and Ontario Regulation 246/22 ss. 70/22 (1).
- Minimum of 2 years’ experience Long Term Care (Gerontology) preferred
- Must demonstrate an active interest in ongoing education
- Must demonstrate good communication skills
- Must be computer literate

- Available 7 days per week

Main Duties and Responsibilities
- Participates actively on the Interdisciplinary Team acting as a resident Advocate
- Assists in interpreting legislation to families and collaborates with the clinical team to implement legislation when required
- Participates in the Admission Care Pathway, from the tour process to the Admission Conference, visiting residents on the waiting list, orientation of new residents and families to the Home
- Offering psycho-social support and assessment during the admission and adjustment phase.
- Maintains complete confidentiality in carrying out all responsibilities associated with the position
- Assists to strengthen communication among residents, families and staff
- Acts as a liaison with Ontario Health atHome and Discharge Planners
- Documents all Social Services activities in the Progress Notes in Point Click Care (PCC).
- Facilitates a monthly Resident and Family Support Group related to identified needs
- Supports both the Resident and Family Councils as required and assigned
- Provides supportive counselling information to families through newsletters and in-service sessions
- Participates in Continuous Quality Improvement Activities measuring outcomes through Admission Surveys, Tour Surveys and the annual program evaluation
- Maintains statistical data and contributes to the collection, use and evaluation of indicator data
- Participates in Teams and Committee meetings including Professional Advisory, Quality Council, Ethics and Pastoral Care
- Provides regular reports on resident support requirements and initiative to the Home Administrator
- Acts as a resource for the Behaviour Management Team, assisting in the education for all staff in managing the resident’s challenging behaviours
- Champions a culture of emotion focused care participating in and supporting the implementation of new care approaches
- Protects own health and health of others by adopting safe work practices, reporting unsafe conditions immediately and attending all relevant in-services regarding occupational health and safety
- Follows all roles and responsibilities for employees and employers as legislated under the Ontario Occupational Health and Safety Act
- Ensures compliance with the provisions of the Occupational Health and Safety Act
- Participates in the orientation process for residents, students, volunteers and staff.
- Organizes and ensures the Resident/Family/Essential Caregiver survey annual process as assigned
- Coordinates and participates in care conferences
