Department: Mobile Acute Care of the ElderlyThe Social Work Geriatric Assessor (GA) is a key member of NBRHC's Mobile Acute Care of the Elderly (MACE) Program, providing specialized assessment and care for older adult inpatients. Using the Comprehensive Geriatric Assessment (CGA) model, the GA conducts multidimensional evaluations and collaborates with physicians, nursing, and allied health professionals to develop individualized care plans that maintain independence, optimize function, and prevent avoidable hospital use. The GA identifies and manages geriatric syndromes, supports early and complex discharge planning, coordinates with community partners, and ensures follow-up for frail or medically complex patients.QUALIFICATIONSEducationBachelor of Social Work (BSW)from an accredited program is requiredCurrent registration in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is requiredCurrent Basic Life Support (BLS) for Health Care Providers (HCP) Certificate is required (recent within the last twelve months)Current Certificate of Registration in good standing as a Health Professional with an Ontario Professional Association or regulatory collegeCompletion of the Provincial Geriatrics Leadership Ontario (PGLO) Common on-boarding (Levels 1 & 2), or willingness to complete within the yearCompletion of Geriatric Certificate Program (McMaster university) required, or willingness to obtain within 2 yearsCurrent Gentile Persuasive Approach (GPA) Certification requiredExperience/Knowledge/Skills:Experience working within a health care setting with a focus on older adults.
Preference is to those with experience within the past 5 yearsExperience or demonstrated understanding of the Comprehensive Geriatric Assessment framework, and geriatric goals of careExcellence in analyzing assessment findings and implementing individualized treatment plans based on the Comprehensive Geriatric Assessment frameworkKnowledge of Specialized Geriatric Services across the continuum - including inpatient services, community clinics, programs, and resourcesKnowledge of consent and capacity matters and legislation applicable to this older adult population (e.G. Public Guardian Trustee, Substitute Decision Maker Act, Health Care Consent Act, Mental Health Act, Long-term Care Act, PHIPA)Ability to exercise professional judgement and critical thinking, with the ability to apply ethical frameworks, supporting the optimal assessment and treatment interventionSkills in management of geriatric syndromes (i.E. polypharmacy, falls, functional decline, acute/sub-acute cognitive changes, bowel/bladder, and nutrition/hydration, etc.)Excellence in a leadership roleExcellent levels of interpersonal skills, written, and verbal communication skills, with consideration of the intended audienceDemonstrate person and caregiver focused care,
