Lead the charge in Early Intervention Hoarding Services as a Supervisor with VHA Home HealthCare. Oversee dedicated coaches and volunteers supporting vulnerable clients in maintaining independent living.
This temporary full-time role reports to the Senior Manager of Responsive Community Care. You will train and supervise a team committed to aiding clients facing hoarding challenges, while conducting assessments to create tailored care plans. Encouraging a supportive setting, you'll also manage case responsibilities and respond to community inquiries about VHA's services.
Key Responsibilities:
• Conduct initial assessments and establish care plans with clients
• Supervise and guide Hoarding Support Coaches and Volunteers
• Input client data and documentation accurately
• Facilitate outreach and community support initiatives
• Ensure compliance with VHA policies and confidentiality standards
Requirements:
• BSW or equivalent in Social Services
• 3-5 years with mental health client populations
• 1-3 years of supervisory experience
• Proven case coordination skills
• Excellent written communication for grant proposals
