Ontario Health atHome is seeking a Care Coordinator to assess patient needs, determine service eligibility, and connect patients with community resources. You will engage with patients, caregivers and health-care practitioners to develop and monitor individualized care plans. You will coordinate care across office, community, or care facility settings, leading a multidisciplinary approach and ensuring patient-centered outcomes. The role is permanent full-time with solid growth and team support.
