We’re seeking an experienced Care Planner & Relationship Coordinator to join our team.
Additional Earning Potential: Up to $20,000 annually through:
- Client Retention & Feedback Bonus - Job Type: Full-Time, Permanent Schedule: Monday to Friday, 40-44 hours per week (occasional evening/weekend flexibility required for operational emergencies) Care Planning & Quality Assurance (Fieldwork 30-50%)
Conduct initial in-home client assessments to evaluate health needs, safety, and personal preferences. Develop, implement, and routinely update highly personalized Care Plans. Perform regular quality assurance (QA) visits to client homes across the Hamilton-Halton-Peel region to monitor service delivery. Maintain detailed, accurate documentation of all assessments, visits, and care plan modifications. PSW Hiring, Training & Supervision Participate actively in the recruitment, screening, and interviewing of top-tier Personal Support Workers (PSWs). Implement ongoing training programs and provide continuous on-the-job education and support to the care team. Evaluate PSW performance in the field, ensuring strict compliance with healthcare standards and agency best practices. Scheduling & Care Coordination Oversee and optimize client-staff scheduling to ensure reliable, consistent, and uninterrupted care coverage. Provide direct, hands‑on care coverage in the field as a last resort during emergency PSW shortages or last-minute call-outs to ensure client safety and continuity of care. Serve as the primary point of contact for clients and their families, ensuring high satisfaction and trust. Foster a supportive, engaging work environment for PSWs to boost employee morale and retention.

Care professionally at local health fairs, senior expos, and networking groups to elevate brand visibility.
Targeted Field Outreach : Build and maintain a robust database of local partners. Conduct proactive outreach to hospitals, retirement homes, and community centres through in-person visits, calls, and presentations.
Relationship Management: Foster trust‑based referral partnerships with discharge planners, care coordinators, wellness managers, and social workers.
Certification: Must hold a valid Ontario Personal Support Worker (PSW) Certificate. An additional healthcare/business related degree or diploma is an asset.
Experience: Minimum 5 years of hands‑on PSW experience, including 1-3 years in a supervision, lead, or coordination capacity.
Technical Skills: Proficiency in healthcare documentation, Microsoft Office (Word, Excel), and familiarity with CRM or scheduling software platforms. Flawless written and verbal English communication skills; secondary languages are considered an asset. Strong networking, interpersonal, and relationship‑building capabilities.
