Join Canadian Mental Health Association Durham as a Case Manager focused on empowering clients with mental health challenges. Provide personalized support, advocacy, and case management services in a collaborative workplace. As part of the CREATE program, you will assist individuals in daily living activities while enhancing their community independence.
This role requires a compassionate professional dedicated to developing and implementing individualized care plans based on client needs. Key Responsibilities:
- Manage assigned caseload of mental health clients
- Develop and update individualized client care plans
- Implement treatment goals with clients and care teams
- Teach life skills for independent living and wellness
- Advocate for clients and coordinate community resources
Requirements:
- Post-secondary education in mental health or related field
- Minimum one year related experience in case management
- Knowledge of Mental Health Act and psychotropic medications
- Excellent communication and interpersonal skills
- Valid Ontario Driver’s license and dependable vehicle
