- Se préparer et assister à des séances de formation spécialisée plus poussées sur des thérapies spécifiques, données par l’équipe pharmaceutique
- Établir et maintenir des partenariats avec l’équipe pharmaceutique
- Assurer la liaison avec les fournisseurs de soins de santé, les pharmacies et les cliniques de perfusion et d’autres organismes au besoin, afin de coordonner les perfusions, l’administration de médicaments et résoudre les problèmes qui surviennent
- Avec les gestionnaires, participer au processus de formation de nouvelles infirmières gestionnaires de cas, y compris, mais sans s’y limiter, l’observation et la surveillance des appels
- Fournir des commentaires au sujet des protocoles, bases de données, processus et relations entre départements actuels
- D’autres fonctions et tâches peuvent être confiées à l’infirmière gestionnaire de cas de temps à autre, selon les besoins
- Peut être appelé à travailler sur appel ou de garde. Provide support to patients and health care providers by offering information on the Patient Support services, specific diseases, and therapies based on manufacturer provided materials and product monograph
- Act as a liaison between patients, families and health care providers related to medication access. Address general inquiries regarding program services, disease states, or product related queries based on provided materials
- Refer the patient to their healthcare providers for clinical questions beyond program materials
- Follow case-management protocols, including data collection, patient follow-up, therapy disruption/discontinuation monitoring, adverse event reporting and liaison with primary healthcare providers
- Collaborate with the reimbursement specialist and the physician to secure funding for their patient’s therapies and explore alternative funding options
- Ensure patient services are coordinated and tracked, according to KPI’s, resulting in expedited access to therapy
- Participate in specialized training sessions for specific therapies provided by the pharmaceutical team
- Attend pharmaceutical meetings, patient meetings, conferences, and work with patient associations and advocacy groups as needed
- Coordinate with health care providers, pharmacies and infusion/injection clinics for treatment logistics and issue resolution
- Assist in the training process of new Nurse Case Managers and provide feedback on protocols, databases, processes and inter-departmental processes
- The Nurse Case Manager will also be assigned other duties and tasks as required from time to time. Baccalauréat ou diplôme d’infirmier(ère) autorisé(e) (IA), diplôme d’études supérieures ou expérience spécifique de gestion d’un cabinet est préférable, avec cinq (5) années d’expérience en soins infirmiers
- Il est indispensable d'être titulaire d'un diplôme d'infirmier(ère) (RN ou RPN/LPN) et d'être inscrit(e) en règle auprès de l'Ordre des infirmières et infirmiers de la province où l'on exerce. A Diploma in health care or a related field, with a preference for Bachelor’s degree Licensed Nurse (RN or RPN/LPN) with active registration from the College of Nurses in the province of employment is required.
Expérience: Au moins deux (2) ans d’expérience du contexte du remboursement par des tiers publics ou privés ou dans les ventes (non requis, mais un atout), la gestion des soins ou le soutien clinique dans l’industrie pharmaceutique, ou expérience connexe

- Bilinguisme (français et anglais) est préférable;
Experience: Minimum of two (2) to five (5) years in the healthcare, pharmaceutical, insurance or related fields
- Ideal candidates should be nurses with healthcare and case management background in medical/hospital settings, insurance or bio-pharmaceutical industry
- Fluency in French and/or English may be required depending on program and geographic location.
