Contribute to community health at Hamilton Urban Core as a part time Nurse Practitioner. Provide trauma-informed care to support individuals experiencing homelessness and complex health needs. This temporary on-site role involves collaborating within an interdisciplinary team to deliver comprehensive client-centered care.

You will conduct health assessments, diagnoses, treatments, and ongoing disease management, focusing on promoting equitable access to health services for marginalized populations. Key Responsibilities:

- Conduct assessments, diagnoses, and treatments aligned with CNO standards
- Provide chronic disease management and follow-up care
- Collaborate with interdisciplinary team members for holistic care




- Develop and evaluate health education and promotion programs
- Supervise and mentor students and learners in practice

Requirements:

- BScN with Nurse Practitioner certification and Ontario registration
- Broad nursing experience with strong clinical assessment skills
- Experience in trauma-informed and harm reduction care
- Sensitivity to issues impacting clients from diverse backgrounds
- Familiarity with government services for complex needs
