Under the administrative direction and supervision of Management, the Physical Therapist provides physical therapy evaluation, treatment, and/or interventions and oversees the physical therapy for patients.

Integrates physical therapy treatment plan into the Plan of Care to maximize benefits for patients. Graduated after successful completion of a physical therapist education program approved by one of the following: ~ o The Commission on Accreditation in Physical Therapy Education (CAPTE) ~ o An education program outside the United States determined to be substantially equivalent to physical therapist entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or an organization identified in 8CFR212.15(e) as it relates to physical therapists ~• Licensed in the state of Florida -Active BLS for healthcare professionals from the American Heart Association or Red Cross.

Certification of completion of Alzheimer's Disease and Related Dementias Training through the Florida Department of Elder Affairs. ~• Mobile Driver - Valid driver’s license and automobile insurance per Company policy ~• Able to travel and have reliable transportation to meet visit schedule ~• Ability to effectively communicate in English (written and verbal) ~• Flexible schedule to meet the needs of patients ~• navigate stairs and narrow spaces, tolerate heat and lack of air conditioning) in any area Company serves ~• Comply with all State, federal and local government regulations, maintaining a strong position against fraud and abuse.

- Participate proactively in improving performance at the organizational, departmental and individual levels.
- Participate in Quality Assessment and Performance Improvement activities as appropriate for the position. Provides physical therapy evaluation, treatment, and interventions to program patients.
- Provides training, guidance, and supervision to program staff and patients/family members regarding therapy.
- Participates actively in the Interdisciplinary Group (IDG) by providing clinical expertise in team planning, problem solving, and processes.
- Fulfills all administrative requirements including, but not limited to, collaboration with Occupational Therapist, attendance at team and staff meetings, and recording and tracking of physical therapy durable medical equipment (DME).
- Records and signs treatments in patient’s medical record; records, signs, and dates clinical documentation.
- Compensation Pay Rate: $55.00 This position requires consent to drug and/or alcohol testing after a conditional offer of employment is made, as well as on-going compliance with the Drug-Free Workplace Policy.
