Make a meaningful impact in your community as a Transitional Case Manager at the Exeter Site in Ontario. Support individuals with mental health and addiction issues while helping them gain independence. We are seeking a full time Transitional Case Manager to fill a vacancy at our Exeter location. This role involves assessing client needs, setting personalized rehabilitation goals, and providing essential support to enhance their quality of life and community connections. Key responsibilities include crisis management and community resource advocacy to facilitate effective recovery. Key Responsibilities:
Conduct assessments to identify client needs and barriers
Develop tailored rehabilitation plans based on individual goals




Teach life skills to promote independence
Coordinate services and advocate on behalf of clients
Evaluate client progress and make necessary adjustments Requirements:
Diploma or degree in a health or social service discipline
Valid G driver’s license and access to a vehicle
Current Police Vulnerable Sector Check
Experience with mental illness preferred
