This part time permanent role in Toronto demands an RPN who can assess, plan, and evaluate comprehensive care for patients experiencing reliable illness. Responsibilities include skilled documentation and effective communication with the healthcare team while focusing on patient-centered care principles. Your expertise will contribute to improving patient quality of life and dignity throughout their care journey. Key Responsibilities:
Collaborate with RNs to assess patient care needs
Advocate for patients and families during care planning




Collect health data for ongoing patient assessments
Document care processes complying with nursing standards
Report changes in patient status to the healthcare team Requirements:
Practical Nursing Diploma and current RPN registration
Current BCLS certification and good performance record
Experience working with older adults preferred
Strong verbal and written communication skills
