As a Registered Practical Nurse BSO, you will guide multi-disciplinary teams, ensuring high-quality support for residents experiencing responsive behaviors due to dementia and mental health issues. Your leadership will facilitate collaboration with key healthcare professionals and enhance behavioral care plans within the designated long-term care home. This role requires a comprehensive understanding of geriatric assessments and the application of best practices in behavior management.

Key Responsibilities:
• Provide expert clinical support and guidance to LTC staff




• Coach staff on behavior care plans and frameworks
• Conduct clinical assessments for responsive behaviors
• Collaborate with various health care professionals
• Document care plans and maintain accurate records

Requirements:
• Current registration as a Registered Practical Nurse in Ontario
• Minimum one year experience in long-term care
• Training in GPA, U-FIRST, and P.I.E.C.E.S.
• CPR/First Aid certification
• Demonstrated ability to mentor and lead teams
