Mackenzie Health in Canada is seeking a Nurse Practitioner to join the Nurse-Led Outreach Team (NLOT) to provide advanced clinical care for adults in long-term care homes within the Western York Region catchment. The role emphasizes assessment, diagnosis, treatment, education, consultation, and research to improve access to timely, evidence-based care and reduce avoidable ED transfers.
You will collaborate with the health-care team, advocate for clients, and support protected, patient-centered care
