As an experienced disability worker, the Service Coordinator focuses on direct support and assessment for individuals with disabilities. Key Responsibilities:
• Develop and document individualized person-centered support plans
• Encourage individuals to advocate for their own needs
• Find recreational and employment opportunities within the community
• Coach individuals to engage socially and recreationally
• Communicate professionally with families and community members
Requirements:
• Valid Class 5 Driver’s License and vehicle use required
• 2+ years of experience in supportive roles
• Basic computer literacy and reporting skills
• Effective communication and observation abilities
• Knowledge in supporting basic living skills
