Employment Type: Permanent, Part-time .5 FTE position (Working 18.5 hours per week within a wide-ranging schedule)
Location Compensation Compensation Pay Class 15 (range of $29.01 to $36.05 per hour), plus Extended Health and benefits package

Duties include:
Coordinate and facilitate individual and group life enrichment activities in personal homes and in the community, enhancing the quality of life for individuals living with dementia
Coordinate and facilitate supportive living services to clients to support health aging at home
To build trusting relationships with care partners to understand the needs of their loved ones and how to best support their quality of life through person-centered planning
To work collaboratively with other team members to ensure the needs of the clients and care partners are being met
Qualifications Education Rehabilitation Worker Diploma or equivalent (2 year diploma),



Personal Care Aide
Valid Class 5 Driver’s Licence and a reliable vehicle that can be used for work and transporting clients
Valid First Aid/CPR/AED certification, or obtain within three months
TLR/PART training
Experience Experience working with people living with dementia and/or disabilities
Working independently and within a team
Problem-solving, being empathetic, patient and resilient
Ability to work cooperatively with care partners and referral sources
Skills Excellent organizational and time management skills
Strong interpersonal, networking, and relationship-building skills
Computer literacy in Microsoft Office, Outlook, and database including file recordings and statistical reporting
Final Candidates are required to provide satisfactory criminal record checks including a vulnerable sector search.
