Enhance Indigenous health as a Diabetes Nurse Educator at SOAHAC in London, ON. This full time role focuses on culturally safe diabetes education and support for clients.
As part of a multidisciplinary team, the Diabetes Nurse Educator will provide comprehensive education to Indigenous adults managing Type 2 diabetes and pre-diabetes. You will engage in individualized counselling, support lifestyle changes, and develop tailored care plans to improve health outcomes. Your contributions will ensure clients receive culturally rooted, effective support in diabetes management.
Key Responsibilities:
• Provide diabetes education using clinical practice guidelines
• Support clients in goal setting and lifestyle modifications
• Develop individualized care plans in collaboration with clients
• Plan and deliver group education sessions
• Participate in program evaluation and continuous improvement initiatives
Requirements:
• BScN or equivalent and current registration with the College of Nurses of Ontario
• Minimum three years in diabetes education or chronic disease management
• Experience with Indigenous communities is an asset
• Valid driver’s license and reliable vehicle for outreach
• Successful completion of a Vulnerable Sector Police Record Check
