GH-CCMH is also a member of the Thriving Minds Mental Health Access Collaborative with CAMH and SickKids and a core service provider in the Toronto sector of infant, child and youth mental health services funded by the Ministry of Health (MOH). GH-CCMH provides prevention, early intervention and health promotion services across the city, funded by MOH and other sources.As such, the Centre has an ambitious agenda to provide the highest quality, evidence-based care, offering a range of services from prevention and early intervention to counseling and therapy, assessment and intensive supports. Services are offered in person and in virtual format. Find out more about GH-CCMH .We are currently seeking applicants for a skilled Child and Family Therapists for our Community Outreach & Intervention Initiative (COII). The intent of the COII program is to address the needs of clients residing in three priority areas who do not seek or engage with traditional service models (cannot attend an agency site, do not have a safe space at home for virtual care) by enabling a pathway of service that travels to the client or builds access in a secure, appropriate environment (e.G., community site, home).The successful candidate will work in a multi-disciplinary team, and have access to experts in the field, ongoing clinical supervision & support,



as well as training and development opportunities. The Child and Family Therapist will provide direct mental health treatment services, including individual, family and/or group treatment to clients within the context of the family, school, and community. If you are looking for a new meaningful career that will allow you the opportunity to positively impact the lives of children and their families, through a recognized industry leader, this may be the career for you.Work collaboratively with external stakeholders in the community to engage with children, youth and families who otherwise may not connect with mental health servicesProvides individualized, assessment, treatment, and follow-up servicesProvides Single Session therapy with the team as well as other brief services as neededProvides case management, liaison, and referral services for clientsProvides virtual and telephone treatment as needed.Coordinates services with internal and external service providers, requesting and reviewing existing documentation and reports to inform client service and treatment planningCollaborates with the family, the supervisor, and inter-professional teams in the development of a service plan and to support treatment planningFacilitate therapy groups for families with young childrenCompletes all client documentation (including, but not limited to, contact notes, service plans, treatment reports, incident/serious occurrence reports, referral forms, etc.) in a timely and succinct manner meeting agency, professional and legal requirementsEnsures that all required client tracking, and statistical information is submitted according to Agency ProceduresPromotes the direction of the program and Centre by engaging in identifying, developing, and implementing program goal(s), projects, or servicesParticipates in relevant meetings, reports regularly,



and as required to the designated supervisorContributes to the Centre's academic mandate and to the field of children's mental health through training, education, and/or the supervision and evaluation of students/trainees/volunteersMay provide services both in-office and within client's homes and communitiesMay be required to supervise studentsOtherComplies with the organization's vision, policies and proceduresPerforms other tasks as assigned by managementQualifications:Post-graduate Degree in a mental health discipline and a minimum of three years' related clinical experience or equivalent combination of education and experience, MSW is considered an assetMembership in a professional CollegeGood verbal and written communication skills to provide therapeutic services to clients and familiesGood problem-solving skills to analyze complex client issues and recommend solutionsDemonstrated skills in therapeutic engagement and behavioural-based interventionsFirst Aid/CPR, UMAB and Behavioural Management Techniques an assetGood knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint), as well as ability to use office equipment efficiently (telephone, scanner, photocopier and fax)Ability to incorporate values of a culturally competent practice of diversity and inclusiveness in all aspects of workAbility to engage in physical activities related to role play, board games, etc., as well as to perform/implement physical restraints as per training, as requiredAbility to work flexible hours, including evenings and weekendsAbility to travel to various locations, including clients' homesWe are seeking applicants internally and externally.GH-CCMH is an equal opportunity employer; we are committed to diversity and value inclusion.
