Patients can make an appointment with a healthcare provider at the clinic while they continue to remain on the registry to find a family practice.This clinic is collaborative in nature and will consist of family physicians, nurse practitioners, family practice nurses, care team assistant, social worker and pharmacist. It is managed by Nova Scotia Health with administrative support and is fully integrated on a Med Access.Responsibilitiesensure the delivery of comprehensive primary health care services to patients of all ages and stages of health or illnessbe accountable for appropriate workload and practicecontinuum of patient care with health care providers within your practice and/or communityprovide care and coordination of community-based care, secondary and tertiary level heath careoperate within the Bylaws and Rules and Regulations of Nova Scotia Health Authoritywork in collaboration with other team members in the practice to best suit the community’s health needswork with other providers to provide after-hours access and emergency coverage when requiredparticipate in quality improvement and evaluation activitiesutilize an electronic medical record (EMR)
