The STAR Family Health Team provides comprehensive primary health care services across multiple clinics, supporting a diverse team of healthcare professionals dedicated to delivering high-quality patient care.

We are seeking a Social Worker (MSW) to join our interdisciplinary team for a temporary 6 - month term. The role will support patients and families by providing a wide range of mental health supports including assessment, brief counselling and psychotherapy, crisis support, education, advocacy, care coordination, and system/community navigation.

The successful candidate will bring a compassionate, trauma-informed, patient-centred approach and work collaboratively with physicians, allied health professionals, and community partners, in addition to the following required qualifications:
Master of Social Work (MSW) from a recognized university.
Current registration in positive standing with OCSWSSW.
Minimum 3 years of relevant clinical experience,



preferably in primary care or community health.
Experience in counselling, psychotherapy, psychosocial assessment, crisis intervention, and supporting individuals and families.
Knowledge of evidence-informed therapeutic approaches and relevant mental health legislation.
Robust knowledge of local health, mental health, social, and community resources.
Excellent interpersonal, communication, clinical, and organizational skills.
Ability to work independently and collaboratively within an interdisciplinary team.
Proficiency with EMRs and Microsoft Office.
Valid driver's licence and reliable transportation required to travel across clinics.

Pay: $39.80 per hour

Expected hours: No more than 37.5 per week
