"The acronym "P.I.E.C.E.S." represents a holistic approach to assessing all aspects of a client, i.E: Physical, Intellectual, Emotional, Capabilities, Environment, and Social.The incumbent is accountable for planning, implementing and evaluating P.I.E.C.E.S.TM as a model of care across the five Health Authorities and has Program responsibility for sustaining and growing the program to support current and future needs and building capacity by embedding the use of the P.I.E.C.E.S.TM approach in the delivery of care. This includes fulfilling the role of primary coach and mentor to support and sustain the spread of P.I.E.C.E.S.TM across the continuum of care and to ensure consistency, continuity and standard practice.
The PDE works collaboratively with other leaders and educators across southern Saskatchewan to provide education services consistent with the overall P.I.E.C.E.S.TM program direction.In addition to coordinating and promoting the spread of P.I.E.C.E.S.TM, the incumbent assesses learning needs of staff, plans, implements, and evaluates education activities/programs as these relate to the care of elderly clients. In collaboration with the Program Services Manager, CNE Supervisor, other stakeholders such as leadership in continuing care homes, hospital and the SHA the PDE is responsible for assessing learning needs of staff, planning, implementing and evaluating education activities/programs.Human Resources Exemption: NoEducationGraduate from an approved School of NursingCompetenciesComputer skills - BasicClinical data analysis skillsLeadership SkillsExperienceExperience in dementia care within the past (5) yearsExperience working with older adultsExperience using applicable clinical/health assessment toolsProven ability to apply the P.I.E.C.E.S care model will be considered an assetMinimum of three (3) years relevant long term care nursing experienceRequires experience in planning, program design, implementation, and evaluation.Experience in teaching adults, facilitation skills,
