You will join the Community Care, Mental Health & Addictions program, focusing on intensive case management for seniors facing mental health and addiction issues. Your responsibilities will include conducting psycho-social assessments, developing service plans, and providing necessary support to clients and caregivers.

Key Responsibilities:
Conduct psychosocial assessments and care planning

Requirements:
Master's in Social Work (M.S.W.) or related field
Registered and in valuable standing with OCSWSSW
Over 3 years of experience in relevant fields
Deep understanding of geriatric care
Proficiency in trauma-informed care principles
