Make a difference with Wood's Homes as a Youth Systems Navigator, guiding youth aged 12-19 through challenges in the Palliser School Division. This temporary full time role focuses on mental health and well-being support.
This position is a term assignment until December 2024, involving direct navigation for youth facing barriers. You'll engage through face-to-face and virtual meetings, providing guidance in daily living activities, mental wellness, and foster collaborative problem-solving. By becoming a trusted expert in youth population health trends, you will support not only individual clients but also enhance service delivery among community partners.
Key Responsibilities:
• Collaborate with Palliser School Division and mental health providers
• Facilitate systems navigation and case management with youth & families
• Conduct targeted outreach to schools and communities
• Participate in team meetings and training sessions
• Build relationships with youth-serving agencies
Requirements:
• Diploma or degree in Social Sciences or related field
• 2-4 years experience in community or mental health
• Current CPR and First Aid Certification
• Training in trauma-informed care
• Class 5 driver’s license and own transportation
