As a member of the CoAct team you play a key role in resident care planning (interdisciplinary including resident and family) and allow residents to achieve their goals and enhance their quality of life. As a HCA you will assist in providing a clean and safe environment, monitor resident progress, and report relevant information to the Registered Nurse in a timely manner. You will monitor and participate in RAI MDS resident tracking, care plan development and implementation as directed. After demonstrating competence, the HCA will be responsible for providing restricted activities deemed appropriate by the organization based on the resident/patient condition and under the supervision of the Registered Nurse.
