Join Canadian Mental Health Association Durham as a Case Manager focused on empowering clients with mental health challenges. Provide personalized support, advocacy, and case management services in a collaborative environment.
As part of the CREATE program, you will assist individuals in daily living activities while enhancing their community independence. This role requires a compassionate skilled dedicated to developing and implementing individualized care plans based on client needs.
Key Responsibilities:
• Manage assigned caseload of mental health clients
• Develop and update individualized client care plans
• Implement treatment goals with clients and care teams
• Teach life skills for independent living and wellness
• Advocate for clients and coordinate community resources
Requirements:
• Post-secondary education in mental health or related field
• Minimum one year related experience in case management
• Knowledge of Mental Health Act and psychotropic medications
• Excellent communication and interpersonal skills
• Valid Ontario Driver’s license and dependable vehicle
