Key Responsibilities:
• Assess care needs and determine eligibility for services
• Develop individual care and service plans collaboratively
• Coordinate and monitor delivery of care plans
• Establish supportive relationships with patients and families
• Ensure patient values and preferences are prioritized
Requirements:
• Membership in a relevant regulatory body
• Minimum of 1 year experience in community health
• Excellent communication and assessment skills
• Valid driver’s license and reliable vehicle
• Proficiency in Windows environment
