About the Role Our client is seeking a proactive and caring Community Support Facilitator to join their Community & Social Care team based in Kimberley . This role is dedicated to assisting individuals and families within the community by providing practical support, guidance, and connection to essential services. You will work closely with clients to understand their needs, help them develop plans for self-sufficiency, and facilitate access to resources such as employment, housing, and healthcare. This hybrid position allows for a blend of community-based fieldwork and remote administrative tasks, offering a flexible and impactful career opportunity within the Northern Cape .
Key Responsibilities Conduct client assessments to identify social, economic, and personal needs.
Develop and implement individualized support plans tailored to client goals.
Facilitate client access to community resources, including social services, educational programs, and employment opportunities.
Provide ongoing support and follow-up to ensure clients are meeting their objectives.




Maintain accurate and confidential client records using both digital and paper-based systems.
Collaborate with community partners and agencies to enhance service coordination and effectiveness.
Requirements Relevant qualification in Social Work, Community Development, or a related field.
Minimum of 2 years of experience in a community support or case management role.
Strong knowledge of local resources and support networks in Kimberley .
Excellent interpersonal and communication skills, with the ability to connect with diverse individuals.
Organizational skills with the ability to manage a caseload and maintain detailed records.
Comfortable working both in the field and remotely, utilizing digital tools.
