As a Care Coordinator (Status: Full-Time, Start date: Immediately; Location: Parry Sound -West Parry Sound Health Centre) , you will assess and determine patient careneeds and eligibility, provide access and referrals to community services, andengage with patients, caregivers and other health care practitioners.
Whether you work in our office, in the community, or a health carefacility – you will play a lead role in providing connected, accessible,patient-centered care – and be supported by our collaborative team thatincludes over 9,000 regulated health care and other professionals.

What will you do?
In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans
Linkpatients with service providers
Coordinate and monitor care plan delivery
Establish a helping relationship with patients and their families
Balance patient needs andchoices with available resources, ensuring patients’ values and preferences arerespected
Initial assignment Hospital, withpossibleassignmentchanges within the occupation based on operational needs
What must you have?
College of Nurses of Ontario
College of Physiotherapistsof Ontario
College of OccupationalTherapists of Ontario
Ontario College of SocialWorkers and Social Service Workers
Recentexperience in community health or a related field
Knowledgeof the health care delivery system and community resources
Excellentinterpersonal, communication, assessment, problem-solving anddecision-making skills
Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
Establishedability to accurately complete required documentation, reports and forms
Avalid driver’s license and access to a reliable vehicle
Advance oral and writtenproficiency in English is essential

What would give you the edge?
Experience working with diversepatient groups, e.g., multicultural, homeless,
palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics
Casemanagement experience or recent related community experience
Abilityto speak French or another second language
Salary:
Hourly wage of $41.29 to $51.13
