Department: CartierThe Alternate Health Facility is 40 bed unit located off site. The AHF will care for patients who require transitional care or an alternative destination of care whose acute care needs have been met, but may require transfer to rehabilitation, long‐term care, a retirement home or other post‐acute care services. Patients at the AHF will be cared for by an inter professional care team of physicians, nurses and personal support workers, in conjunction with dietary, environmental services, and support from the Infection Prevention and Control Practitioner.Equipped for acute medicine patients, the AHF site offers the same quality of care as NBRHC main siteCertification/Experience/Knowledge:Ontario Secondary School Graduation Diploma or Secondary School Graduation Diploma or equivalent requiredPersonal Support Worker Certificate, HealthCare Aide Certificate or completion of minimum second (2nd) semester in a recognized nursing program requiredCurrent Basic Life Support (BLS) for Health Care Providers (HCP) Certificate required (recent within the last twelve months)Dementia care, pharmacotherapy, falls/mobility, osteoporosis, delirium training requiredDe‐escalation techniques requiredGentle Persuasive Approach (GPA) certification an assetSkills/AbilitiesExcellent level of interpersonal skills requiredExcellent level of written and verbal communication skills requiredAbility to work as a cooperative member of the health team requiredAbility to work with minimal supervision requiredAbility to meet the physical demands required to provide care to medical patients requiredDemonstrated ability to engage in meaningful patient activity is requiredDemonstrated ability in flexible care and patient centred approaches is requiredCritical thinking, problem solving, ability to delegate, multi task is requiredMust be a motivated, flexible and organized individual with patience, tact and an understanding of the special needs of the elderlyDemonstrated commitment to continuing education requiredBasic computer skills requiredMust demonstrate commitment to ensure your safety, the safety of fellow workers, patients, volunteers and visitorsMust support and contribute to a culture of safety and prevention of adverse health events in this organizationGood attendance and work record requiredBilingualism (Advanced level English/French) is required.
