Position: Care Coordinator RN, PT, OT or SW #072-26 CARE AND BE CARED FOR – THIS IS YOUR HOME Are you an experienced registered nurse,physiotherapist, occupational therapist, or social worker seeking a rewarding career that cares for others, in aprofessional practice that cares for you? As a Care Coordinator (Full-Time, Permanent; Start date: Immediately; Location: Sudbury) , you will assessand determine patient care needs and eligibility, provide access and referralsto community services, and engage with patients, caregivers and other healthcare practitioners. Whether you work in our office, in the community, or a health carefacility – you will play a lead role in providing connected, accessible,patient-centered care – and be supported by our collaborative team thatincludes over 9,000 regulated health care and other professionals.
A s a valuedteam member, yourmission will be to help our patients be healthier at home, while you benefit from oursupports for skilled growth, personal wellness and work-life balance. In collaboration with patients and their families, assess careneeds, determine eligibility for services,
and develop individual care andservice plans Coordinate and monitor care plan delivery Establish a helping relationship with patients and their families College of Nurses of Ontario Ontario College of SocialWorkers and Social Service Workers Recentexperience in community health or a related field Knowledgeof the health care delivery system and community resources Advanced oral and writtenproficiency in English essential We have amandatory COVID-19 vaccination policy.
