Position: Care Coordinator RN, PT, OT or SW #036-26 CARE AND BE CAREDFOR – THIS IS YOUR HOME Are you an experienced registered nurse,physiotherapist, occupational therapist, or social worker seeking a rewardingcareer that cares for others, in a qualified practice that cares for you?As a Care Coordinator (Full-Time, Startdate: Immediately; Location: North Bay or Kirkland Lake) , you willassess and determine patient care needs and eligibility, provide access andreferrals to community services, and engage with patients, caregivers and otherhealth care practitioners. Whether you work in our office, in thecommunity, or a health care facility – you will play a lead role in providingconnected, accessible, patient-centered care – and be supported by ourcollaborative team that includes over 9,000 regulated health care and otherprofessionals. In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans Coordinate and monitor care plan delivery Establish a helping relationship with patients and their families Membership,in good standing, with the applicable regulatory body:
Collegeof Physiotherapists of Ontario Collegeof Occupational Therapists of Ontario OntarioCollege of Social Workers and Social Service Workers Recentexperience in community health or a related field Knowledgeof the health care delivery system and community resources Advancedoral and written proficiency in English and French is essential Wehave a mandatory COVID-19 vaccination policy. As a condition ofemployment, all employees are required to submit proof of COVID-19vaccination status prior to start date.
