JOB SUMMARY The Windsor Family Health Team’s (WFHT) Mission is Providing Exceptional Primary Care as an Integrated Team.

Reporting to the Director of Clinical Operations, the Clinic Nurse (Ontario Health funded position of Registered Practical Nurse) provides direct client care and related support, including client and family health education and counseling.

This position is an existing vacancy within the organization.

EDUCATION AND TRAINING

- In good standing with College of Nurses of Ontario
- Current CPR certification

QUALIFICATIONS AND EXPERIENCE

- Minimum 1-year work experience in a primary care setting.

- Experience working under medical directives.
- Knowledge of client assessment and triage procedures.
- Ability to perform standard clinical procedures according to established protocols (i.e. injections, suture removal, dressing changes).

- Ability to maintain patient flow.
- Ability to provide health teaching to patients and/or families as appropriate.
- Ability to follow and maintain quality, safety, and/or infection control standards.

- Ability to complete accurate client charting in accordance with CNO documentation standards and organizational policies.
- Motivational interviewing skills.
- Knowledge of inventory management practices.

- Experience working in a multidisciplinary team while also working well independently.
- Knowledge of local community agencies to assist/link patients to access beneficial supports.
- Experience using an electronic medical record.

- Understanding of privacy legislation and its application within a primary care setting.

- Works within and to the full scope of practice of a Registered Practical Nurse.

- Assists with direct client care procedures and related tasks; checks in clients, obtains client subjective and objective assessments, takes vital signs, prepares charts and assists with medical examinations.
- Performs routine specialized procedures, such as, sterile dressing changes under established protocols.
- Maintains stocks of vaccines, sample medication and other supplies as necessary.





- Educates and advises clients on specified medical issues within established parameters and scope of practice.
- May perform specialized procedures, such as processing of lab specimens, under strict protocol and procedural guidelines.
- May coordinate office activities and perform a range of accounting and administrative functions in support of unit activities.

- May oversee/mentor student clinical experiences.
- May recruit clients for studies as required.
- Practices safety, environmental, and/or infection control methods.
- Ability to safely perform and support client transfers using mechanical lifts (e.g., Hoyer lift), following appropriate training and organizational procedures.

- Moderate physical activity required.
- Proactively contact clients to educate them on recommended preventative health screenings and support their participation in appropriate screening programs.
- Requires handling of average-weight objects up to 25 pounds.

- Requires standing and/ or walking for more than 4 and up to 8 hours per day.
- Attends and participates in meetings and on committees.
- May be required to submit annual TB and rubella screening.

- Performs miscellaneous job-related duties as required.

- complies with WFHT’s Policies and Procedures, applicable legislation as well as professional standards of conduct in human resources and financial management;
- is cooperative with other staff members both individually and in teams;
- respects and values the diversity of communities and individuals;

- promotes awareness of and participation in WFHT activities;
- contributes to the WFHT’s work by attending and participating in meetings and committees by contributing ideas, opinions and information;

OTHER CONDITIONS

- Must be able to perform other duties as reasonably assigned.
- Must be able to work evenings and weekends.
- Ability to travel within Windsor-Essex as required.

- Candidates successful in obtaining employment will be required to provide a Criminal Record Check with Vulnerable Sector Screening valid within six months of start date.

Wage at top current Ontario Health funded rate: $26.77 per hour

40 hours/ week including paid 1-hour lunch
