The Community Liaison Nurse role with Vancouver Island Health Authority focuses on coordinating complex discharge planning, assessing client needs, and ensuring continuity of care across acute and community settings. You will work within a patient-centred model and collaborate with an inter-qualified team to optimize safety and quality of care.

Reporting to Regional Clinical Operations, the position requires BD CNMs RN registration, gerontology experience, and a valid BC driver's license.
