Position Type: Full Time & Part Time | Community-Based | Adult Population Joseph's Home Care we’re dedicated to delivering compassionate, high-quality care to clients transitioning from hospital to home. Our interdisciplinary team supports individuals with complex medical needs, helping them regain independence and thrive in their own environments. As a Dietitian,you’ll play a vital role in supporting clients witha wide variety ofspecial dietary needs, variousheath conditions, mental health and/oraddictions and/ormobilityissuesproviding the highest quality of nutritional care to optimize the nutritional status of program clientsin accordance withindividual care plans, SJHC standards, the College of Dietitians ofCanadaand the Ministry of Health guidelines.
This role is ideal for a compassionate, innovative, and collaborative professional who thrives in a dynamic home and community care setting. Assess,monitorand helpmaintainthe nutritional status and requirements of clients, especially those at high nutritional risk. Evaluate the nutritional status and needs of clients as care plan requires Educate clients and staff on therapeutic and general nutrition, asrequired.
Participate in client reviews,palliative care,caseconferencesas a member of the multi-disciplinary health care team. Participate in family care conferences as requested. Evaluate and update system of providing nutritional care.
Participate in practice-based research to further knowledge in geriatric nutrition.
Perform preliminary swallowing assessments,liaisewith community swallowing assessment team asrequired. Advocates for clients asappropriate, supports clients in connecting withcommunity-basedservices as related to food security needs Builds relationships with community agencies, cultural organizations, other supports,participatesin community initiatives Performsgeneral administrative duties related to the role University level degree in a related field (B.Sc, Home Economics, Food and Nutrition etc(required) Completion of a dietitian internship (required) Updated and unrestricted registration with a professional college (College of Dietitians) (required) Experience with mental health / addictions populations ~ Experience working with vulnerable clients in community care settings ~ Familiarity with mobile work tools and EMR systems Creative problem-solving and critical thinking abilities Committed to equity, inclusion, and continuous learning Flexible, community-based work environment Paid Mileage and Travel Time ~% in lieu of benefits and vacation ~ Paid per hour at a negotiated rate ~ We’re Committed to Equity Joseph's Home Care is an equal opportunity employer.
