Ontario Health atHome seeks a detail-oriented Team Assistant to support Care Coordinators in Sudbury or Sault Ste. Marie. You will process referrals, manage patient data, and handle inquiries, contributing to timely, patient-centered service.

The role requires a Grade 12 diploma, 2+ years in an office setting, and proficiency in MS Word/Excel and database tools. Flexibility to work days, evenings, and weekends is needed.
