Outreach and Program Facilitator
TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking one Outreach and Program Facilitator to join our team in a full-time permanent role across East Toronto and Downtown East communities.
The ideal candidate will hold a diploma or degree in Social Services, Community Development, or a related field, with at least two years of experience working with vulnerable clients, preferably seniors. Demonstrated experience in Community Development, outreach, recreational programming, program planning, program facilitation and program evaluation is essential. Candidates should have strong interpersonal, communication, organizational, and problem-solving skills, with a proven ability to work independently and as part of a team. Experience working with at-risk or frail seniors, including those with cognitive impairments, mental health, or mild to moderate medical issues, is highly preferred. Knowledge of therapeutic program planning, dementia programming, and community service settings is an asset.
Candidates must possess excellent computer skills (Word, Excel, databases, PowerPoint or Publisher), and certifications such as Food Handler’s Course, CPR, and First Aid are considered assets. The role requires flexibility to work alternate Saturdays, weekday and evening shifts. A valid driver’s license and access to a reliable vehicle are required.
DUTIES & RESPONSIBILITIES
Community Outreach & Engagement
- Develops and implements outreach strategies to increase awareness, participation, and access to TNO programs and services.
- Conducts outreach in neighbourhoods, community spaces, housing sites, community events, and other appropriate settings.
- Builds and maintains positive relationships with community members, local organizations, service providers, and other stakeholders.
- Identifies emerging community needs, barriers, and service gaps and communicates findings to the appropriate TNO teams.
- Promotes TNO programs, services, events, and resources through community outreach, presentations, information sessions, and other engagement activities.
- Participates in and supports local community events, initiatives, campaigns, and activities.
- Facilitates networking and community engagement opportunities that strengthen connections among residents, service providers, and community organizations.
- Encourages community participation, leadership, volunteerism, and empowerment.
Community Development & Partnerships
- Develops and maintains partnerships with community organizations, service providers, schools, health and social service agencies, cultural organizations, and other relevant stakeholders.
- Participates in neighbourhood-based networks, committees, roundtables, working groups, and community development initiatives.
- Represents TNO at community meetings, sector networks, outreach events, and other relevant forums.
- Hosts and facilitates community meetings, networking sessions, information sessions, and engagement activities.
- Incorporates community development principles into program planning and implementation.
- Identifies opportunities for collaboration, partnership development, and coordinated service delivery.
- Supports community members to develop leadership skills and participate in initiatives that strengthen their communities.
- Contributes to advocacy, community engagement, and empowerment activities aligned with TNO’s mission and program objectives.
Program Planning, Development & Facilitation
- Plans, develops, coordinates, and delivers programs, activities, workshops, and group sessions based on identified community needs and program objectives.
- Independently prepares, sets up, facilitates, and evaluates programs and activities with minimal supervision.
- Develops monthly work plans, program schedules, outreach plans, and activity plans.
- Designs and facilitates engaging, inclusive, accessible, and culturally responsive programs and group activities.
- Develops and implements new programs and initiatives in response to emerging community needs.
- Plans and coordinates special events, information sessions, workshops, and community activities.
- Prepares and evaluates program materials, resources, presentations, and facilitation tools.
- Ensures program spaces, equipment, and materials are prepared before activities and appropriately organized and cleaned up afterward.
- Monitors participation and feedback and makes recommendations to strengthen program quality, accessibility, and participant engagement.
Participant Support, Information & Referrals
- Establishes and maintains respectful, supportive, and skilled relationships with program participants, community members, families, caregivers, and service providers.
- Conducts program orientations and provides information regarding eligibility, registration, available services, and community resources.
- Identifies participant and community needs and provides appropriate information, referrals, and connections to internal and external services.
- Acts as a liaison between participants and relevant TNO programs, departments, and external service providers.
- Provides advocacy and support to help participants navigate community and social services.
- Responds to participant questions, concerns, and feedback in a timely and respectful manner.
- Supports participants in identifying and accessing resources that promote social connection, independence, well-being, and community participation.
- Maintains appropriate professional boundaries and ensures participant confidentiality and dignity.
Safety, Accessibility & Participant Well-Being
- Promotes a safe, welcoming, inclusive, and accessible environment for all participants and community members.
- Supports the physical safety, emotional well-being, dignity, and inclusion of participants during programs and outreach activities.
- Considers relevant Social Determinants of Health when identifying participant needs and connecting individuals with appropriate supports.
- Ensures compliance with TNO health and safety policies, procedures, accessibility requirements, and other organizational standards.
- Identifies and appropriately responds to safety concerns, incidents, or situations requiring additional support or referral.
- Documents participant concerns, incidents,
and follow-up actions in accordance with TNO policies and procedures.
Administration, Documentation & Reporting
- Maintains accurate and timely participant, outreach, program, and administrative records.
- Inputs required information into appropriate databases and information systems.
- Tracks program attendance, outreach activities, referrals, partnerships, participation, and other relevant program data.
- Prepares and submits required weekly, monthly, and other program reports within established timelines.
- Collects and documents participant and community feedback to inform program planning and continuous improvement.
- Maintains current program resources, outreach materials, referral information, and community resource directories.
- Ensures documentation is completed in accordance with TNO policies, procedures, privacy requirements, and program standards.
Teamwork & Organizational Collaboration
- Works collaboratively with TNO departments and programs to coordinate services and strengthen integrated service delivery.
- Shares information, resources, community connections, and emerging needs with relevant TNO teams.
- Participates actively in team meetings, planning sessions, case discussions, training, and organizational initiatives.
- Supports colleagues and contributes to a respectful, inclusive, and collaborative workplace.
- Works with the appropriate program team to achieve successful participant and community outcomes.
- Performs other related duties as assigned in support of program and organizational objectives.
Screening – As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.
Job Type: Full-time permanent position.
Work Location(s): 45 Overlea Blvd. Toronto, ON M4H-1C3, 10 Gateway Blvd, Toronto, ON, M3C 3A1 and other locations base on program needs.
Schedule: 35 hours per week, including two Saturdays per month, early morning shifts, and some evening hours as required.
