The Dufferin Area Family Health Team currently has an opportunity for a full-time Primary Care Patient Attachment Nurse Coordinator to join our compassionate, collaborative, multi-disciplinary, dynamic health care team dedicated to supporting patients for improved health and well being. The successful candidate will be an effective navigator and connector who can assess the needs of unattached patients, prioritize and match them to primary care resources, build strong relationships with local providers, and coordinate the patient journey through to successful attachment. They will bring a systems-thinking mindset, excellent communication skills and a demonstrated ability to work collaboratively across primary care, and community settings.

The Primary Care Patient Attachment Nurse Coordinator plays a crucial role in ensuring timely and equitable connection to a primary care provider.

Key Responsibilities

- Assessment and Coordination: The role provides supportive intake and assessment,



connects patients to appropriate health and social support while awaiting successful attachment to a primary care provider.
- Facilitating Patient Attachment: The coordinator works closely with clinics to match patients with family physicians when there is capacity. The process is achieved through accessing patients that have registered on Access primary care Dufferin https://hillsofheadwaterscollaborative.ca/accessprimarycare/
- Community Engagement: The role includes establishing and maintaining connections with health and social care providers, working collaboratively to support the patient attachment processes
- Administrative Support: The coordinator provides administrative support, including maintaining accurate clinical information in the OCEAN registry and Telus PS Suites EMR.
- Collaboration with Providers: The coordinator collaborates with Primary Care Providers, Clinic Managers and patients to support attachment.

As a Primary Care Patient Attachment Nurse Coordinator, key aspects of your role will be: · Clinical intake and assessment

· Prioritization of patients

· Matching patients to the right provider

· Preparing the patients and provider for handoff

· Support patients that are not immediately attachable, with system navigation or creating other linkages as appropriate

· Building and maintaining relationships with provider and partners

Required Education and Skills

· Registration with the College of Nurses as either an RN or RPN

· Current knowledge of community and health services within Dufferin-Caledon

· Demonstrated ability to work independently, take initiative, be flexible, and collaborate well with others

· Excellent communication skills

· Strong clinical assessment skills

· Systems thinker

· Strong judgement and discretion

· Criminal Records Check/Vulnerable Sector Screening

Hourly Rate: $28.56 - $39.83

This is a hybrid role

Office location is Orangeville-Caledon

Hours are 37.5 hours per week – including one evening shift per week

Vehicle for travel to sites and within community
