OPTIONS Northwest is seeking a compassionate, skilled, and collaborative Registered Nurse to join our Community Resource Team as a Developmental Services Nurse. This rewarding role supports people with developmental disabilities and/or dual diagnoses by promoting health, wellness, independence, and quality of life within the community.
As a member of a multidisciplinary team, you will provide nursing consultation, assessment, education, advocacy, and direct clinical support to people supported, families, support workers, and community partners. Your expertise will help ensure people r receive person-centred, evidence-based healthcare and access to the services they need to thrive.
What You'll Do
Clinical and Community Support
- Conduct comprehensive health assessments and nursing consultations.
- Develop, implement, coordinate, and monitor healthcare supports and interventions.
- Provide direct nursing care in accordance with College of Nurses of Ontario (CNO) standards and applicable legislation.
- Support individuals and their teams during complex health situations, including hospital transitions, palliative care, end-of-life care, and crisis situations.
- Participate in Community Resource Team meetings, care conferences, and person-centred planning meetings.
- Collaborate with physicians, nurse practitioners, pharmacists, hospitals, home and community care providers, and other healthcare professionals.
- Advocate for people and help address barriers to healthcare access.
Education and Capacity Building
- Provide health education and consultation to people we support, families, and support staff.
- Design and facilitate training programs for staff, community agencies, and the public.
- Promote health awareness, disease prevention, in developmental services.
Documentation and Reporting
- Maintain accurate, timely, and comprehensive clinical documentation.
- Complete health assessments, case formulations, consultation reports, progress notes, and service records.
- Ensure documentation meets organizational, professional, and legislative requirements.
Professional Practice
- Maintain professional competency and nursing registration requirements.
- Participate in ongoing education and professional development.
- Contribute to a culture of continuous improvement and evidence-based practice.
What You Bring
Required Qualifications
- Graduate of an approved Nursing Program.
- Current registration and good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN).
- Experience supporting people with developmental disabilities and/or dual diagnoses.
- Demonstrated ability to provide adult education, develop health-related initiatives, and support caregivers in community settings.
- Current First Aid and CPR certification.
- Ability to obtain and maintain Nonviolent Crisis Intervention (NCI) certification.
- Excellent communication, interpersonal, and relationship-building skills.
- Valid Ontario driver's licence and access to a reliable vehicle.
- Proficiency with Microsoft Word, Outlook, and electronic documentation systems.
- Current Police Records Check (required upon conditional offer)
Assets
- Nursing Foot Care Certification.
- Experience working within developmental services, community health, mental health, or dual diagnosis settings.
