Duties

- Deliver comprehensive patient assessments and develop personalized care plans to meet individual health needs.
- Administer medications, injections, and treatments accurately following medical protocols and physician orders.
- Utilize electronic health record systems such as Epic and Cerner to document patient information meticulously.
- Monitor vital signs, manage pain, and provide wound care to promote recovery and comfort.
- Collaborate with multidisciplinary teams including physicians, therapists, and case managers to coordinate effective patient care.
- Conduct phlebotomy procedures for blood draws and laboratory testing to support diagnostic processes.
- Assist with patient education on medication management, health maintenance, and disease prevention strategies.

Experience

- Proven experience working in hospital settings with acute care responsibilities.
- Strong background in medical terminology, medication administration, and patient care procedures.
- Prior experience with Epic or Cerner electronic health record systems is highly desirable.
- Hands-on experience in pain management, dementia care, senior care, or pediatrics is a plus.
- Familiarity with home care services or assisted living environments enhances your ability to deliver versatile nursing support.
- Laboratory experience including phlebotomy techniques to support diagnostic testing.
- Knowledge of medical office operations and medical terminology ensures seamless communication within healthcare teams.

Position: Registered Nurse

(Permanent position, Scarborough)

Nurse Practitioner-Led Clinics are a primary health care delivery model in which Nurse Practitioners are the lead providers of primary health care. Our clinic will provide service to those clients who do not have a primary health care provider or choose to use the clinic for all of his or her primary care. Clients are offered a variety of services through an interprofessional team of Nurse Practitioners, Registered Nurses, Collaborating Physicians, Physiotherapists, Dietitians, Social Workers and Health Promoters.

Our clinic is focused on improving quality of care by integrating primary care with mental health services, health promotion, case management,



disease prevention and chronic disease management.

Position: 1 FTE, Registered Nurse The Registered Nurse works in collaboration with nurse practitioners, members of interprofessional team members to provide patient-centred, quality primary health care services. Youwill work closely with our interprofessional team to support the attachment of patients to our clinic. You will have chance to support the development of services as part of our expansion.

The Registered Nurse will practice within his or her scope of practice as outlined in the Standards of Practice by the College of Nurses of Ontario.

Major Job Responsibilities:

· Assess patient health care needs (physical, emotional and psychological) to identify appropriate nursing interventions

· Analyze findings of health assessments

· Monitor, record and report symptoms and changes in patients’ conditions

· Collect data through therapeutic relationships with patients

·Treatment and Management o Administer medications and treatments as prescribed by a physician/nurse practitioner o Telephone triaging when needed o Administer vaccinations o Blood pressure assessments o TB skin tests o Foot assessments / Foot care o Assist Nurse Practitioner with assessments and interventions

·Education and Advocacy o Help patients identify and use health resources, including those available in the community and from community agencies o Involve patients in decisions about their own health o Encourage patients to take action for their own health o Initiate health education and other activities that assist, promote and support patients as they strive to achieve the highest possible level of health

·Referrals and Collaboration o Collaborate with members of interprofessional team to plan, implement, coordinate and evaluate patient care in consultation with patients and their families o Consult and interact with other service providers as needed

·Administration and Reporting o Coordinate and expedite clinic flow by triaging patients o Provide Health Education in person and by telephone.

o Arrange follow-up appointments as necessary o Collect accurate and comprehensive patient-related information and document same in the patients' Electronic Medical Record.

o Participate in Quality Improvement Initiatives if needed o Operate and monitor medical apparatus or equipment and ensure strict adherence to sterilization procedures o Handle and dispose of specimens and/or objects, and practice general safety procedures at all times o Monitor supply levels and/or expiration of medical and clinic supplies,



and order supplies as needed from approved vendors o Promote awareness of the Clinic services and programs.

o Participate in the development, implementation, monitoring and evaluation of treatment, education, counseling and health promotion programs and services for individuals, families and the community.

· Participate in clinical projects/studies as required.

· Participate in staff development, training and meetings as required

· Perform other related duties as required.

Skills & Qualifications:

- Member in good standing with the Ontario College of Nurses.
- A satisfactory result of Vulnerable Sector Police Record Check is a requirement of this position.
- 3-5 Years of primary health care experience
- Excellent communication skills
- Able to work autonomously with minimal supervision
- Able to work effectively with all members of the inter-professional team
- Able to prioritize, manage time effectively and be flexible in a very active work environment
- Experience with EMR, Telus PSS would be an asset.

Work Conditions:

1. 1 FTE, 37.5 hours per week.
2. Travel within the community may be required.
3. Able to work flexible work hours, including some evenings and weekends
4. Work in all NPLC locations when needed.

Compensation: Salary$36-39 per hour
