About Us

Bayview Seniors Community is a licensed long-term care home in White Rock, BC, regulated under BC's Community Care and Assisted Living Act and Residential Care Regulation. We provide safe, person-centred care to seniors and are looking for an experienced, organized and caring Resident Care Coordinator to lead day-to-day clinical care.

Position Details

- Status: Full-time, permanent
- Schedule: Monday to Friday, [08:00–16:00] [plus participation in the management on-call rotation, if applicable]
- Reports to: [Director of Care / Executive Director]

Position Summary The RCC is the designated employee who supervises care staff, coordinates and monitors resident care, and manages unusual situations and emergencies, as required by Section 41 of the Residential Care Regulation. The RCC makes sure every resident receives secure, consistent care that follows their individual care plan and respects their health, safety and dignity.

Main Responsibilities

Resident Care

- Monitor residents' health status, changes in condition, falls, wounds, responsive behaviours and care needs.
- Assess residents and make sure physicians, nurse practitioners and families are notified promptly of illness, injury or significant change.

Care Plans

- Develop, review and update individualized care plans, including on admission, after a change in condition and at least annually.
- Make sure staff know and follow each resident's care plan.

Staff Supervision

- Guide, supervise and support RNs, LPNs and Health Care Assistants (HCAs/RCAs).
- Make sure assignments, workload and staffing are appropriate to residents' needs.
- Make sure staff only perform duties they are trained and competent to do.
- Take part in performance reviews.

Family Communication

- Communicate with residents, families and representatives about care concerns, changes in condition and care conferences.




- Respond to concerns and complaints according to Bayview's dispute resolution policy.

Interdisciplinary Coordination

- Work with physicians, nurse practitioners, pharmacy, physiotherapy, occupational therapy, dietitians, social workers and other providers.
- Coordinate referrals, orders and follow-up.

Admissions, Discharges and Transfers

- Take part in pre-admission screening and assessments, to confirm the home can provide safe and adequate care.
- Coordinate admissions, discharges, hospital transfers and returns.

Incident Management

- Follow up on falls, medication incidents, injuries, responsive behaviours and other unexpected events.
- Make sure reportable incidents are reported immediately to families, physicians and Fraser Health Licensing, and are documented in PointClickCare (PCC) Risk Management.
- Lead post-incident review and action planning.

Documentation

- Review clinical documentation in PCC, including assessments, progress notes, MARs and care plans.
- Make sure records are accurate, complete, current and kept confidential.

Quality and Compliance

- Help maintain licensing standards and Residential Care Regulation requirements.
- Support infection prevention and control, outbreak management, medication safety, restraint minimization, falls prevention and emergency preparedness.
- Take part in audits and quality improvement activities.

Education

- Orient new nursing and care staff.
- Provide ongoing education.
- Coach staff when practice issues come up.

Care Conferences

- Schedule and lead resident/family care conferences.




- Document the outcomes and make sure follow-up actions are completed.

Qualifications

- Registered Nurse with current practising registration with the BC College of Nurses and Midwives (BCCNM)
- Minimum 5 years of nursing experience in Canada, including recent experience in long-term care or residential care
- Leadership or supervisory experience (e.g. charge nurse or coordinator) is required
- Strong knowledge of gerontological nursing, dementia care, falls prevention, wound care, palliative care and medication management
- Working knowledge of the BC Residential Care Regulation and Fraser Health Licensing requirements
- Experience with PointClickCare and MDS/RAI assessments is an asset
- Excellent communication, organizational and problem-solving skills

Conditions of Employment As required for licensed care facilities in BC (Residential Care Regulation, s. 37), the successful applicant must provide:

- A clear criminal record check through the BC Criminal Records Review Program
- Professional references, including a recent supervisor
- Proof of current BCCNM registration and copies of education credentials
- Proof of immunizations and TB screening
- A current first aid and CPR certificate (or willingness to obtain one)
