General Accountability and Key Responsibilities:
The CPH Senior Home Supports Leader is directly accountable to their Home Supports Coordinator and indirectly to the Manager of Support Services & Executive Director.
He/she shall be directly accountable for providing high quality support to people, working together with their support team and Home Supports Coordinator to ensure that goals and objectives are met. Specifically,
● Provide direct support to individuals with intellectual disabilities
● Ensure the individuals supported are living the highest quality of life.
● Ensures the safety and wellbeing of the individuals supported.
● Ensures medication is being administered correctly and individual prescriptions filled
● Provides support and guidance to the Home Supports Team
● Coordinates training and performance evaluations of Support Personnel.
● Financial Management of home (i.e. Expenses, Payroll/timecards)
● Networking among the individuals personal network
● Ensured CPHs philosophy is being adhered to by all Support Personnel
● Actively participates in monthly Leadership Meetings
● Participates in annual Support Plan Meetings
● Coordinates monthly Team Meetings
● Maintains ongoing communication with the Home Supports Coordinator, Manager of Support Services, and Executive Director.
● The ability to manage and respond to crisis situations that may arise
Nature and Scope:
This position reports directly to the Home Support Coordinator of CPH. Each team has approximately 5-10 full-time and part-time employees reporting to this role.
The SHSL liaises frequently with the Home Supports Coordinator or the Manager of Support Services & Executive Director. Good relations must be developed and maintained with the people we support, their network (family, friends, and personal networks), CPH management personnel and other co-supports at CPH. Good external relations must be developed and maintained with Social Workers, health care professionals, agencies and community employers.
The most important decisions of the CPH Senior Home Supports Leader relate to quality of life and support provided to individuals particularly:
● Ensuring that planning is done with the individual and network
● Ensuring the service provided is of high quality and in harmony with planning and CPH philosophy.
● Always working toward the individuals overall satisfaction.
The Team
Leader understands he/she is coming into the home/life of individuals served by CPH. The recognition of this must remain foremost in our minds. We must therefore respect their wishes in their service provision within the parameters of safety and quality of life. These parameter must be decided at the team level with the support of the individual, the family/personal network, funding bodies and community supports (where available).
The hiring, training and development of employees is done in consultation with the CPH Home Coordinator and the CPH Manager of Support Services.
The Authorities of this position includes:
1. Making personnel recommendations relevant to the assigned support team, in consultation with the individuals supported to the MSS.
2. Developing and supervising direct service to all individuals served by CPH.
3. Managing budgeted support hours and program costs relevant to the team.
4. Primary responsibility for direct support team orientation. Principal Functions and Accountabilities:
1. Provides direct support and lifestyle planning consistent with CPH beliefs and values and Gentle Teaching.
2.
Provides direct support to people relevant to their personal plan and contract expectations.
3. Provides direct support that reflects the Gentle Teaching Principles, in that: i) Intent is not in changing others behavior but in changing ourselves ii) Support is always focused on teaching the person to feel safe and loved and become more loving and engaged.
iii) Positive use of the 4 tools in everyday support (hands, eyes, words, presence).
4. Support to people may include: i) Ensures evaluation of the satisfaction of individuals and their network through regular contact and open communications with individuals, families support networks and employees.
ii) Liaises and participates with social workers as required to ensure that individual planning and review occurs, service needs are identified and met, and individual and personal network concerns are mediated and resolved.
iii) Assists with updates and maintains CSS’s for all individuals iv) Liaises and cooperates with each person’s day program concerning individual’s issues, needs and desires.
v) Works within the community to develop options relevant to the individual’s plan (i.e. Special Olympics, Friendship Club, etc…).
vi) Liaises with health care professionals as needed, regarding individuals health needs, coordinates and attends health care appointments and maintains records of appointments and outcomes. Ensures implementation of recommendations through the individuals support team.
5. Team Work: i) Facilitates open and honest communication on a regular basis with their Home Support Coordinator, Manager of Support Services & Executive Director, those we support, their families, social workers, support networks and CPH employees.
ii) Is available to the support team via cell phone and at times in person as needed.
iii) Ensures continuity of support in circumstances such as crisis response and support worker shortage.
6. Leadership responsibilities include: i) Provides support schedules for the support team, which will ensure quality support for the individuals. Posts schedules at least (2) two weeks in advance.
ii) Verify the accuracy of time sheets against the schedule before submitting them for payroll.
iii) Double checks and submits time and expense sheets for their team to the office as per schedule.
iv) Coordinates and leads Team meetings, at least monthly. Invites Home Support Coordinators and MSS/Executive Director to.
v) Ensures proper accounting and reporting of finances for the support as well as any personal funds individuals may entrust to them.
vi) Coaches, mentors, and assists employees in providing excellent individualized service including evaluating employee performance, building an effective team, developing employee performance plans, identifying training needs, applying corrective discipline as required, and participating in conflict resolution.
vii) Ensures the support and equipment used meets safety requirements.
viii) Adheres to requirements outlined in contracts and applicable legislation. Reports to social workers and other stakeholders as required.
ix)
Is an active member of the CPH Leadership meetings by attending all scheduled meetings, providing input and ideas, attending in- services, functions, team building activities and other as requested.
x) Ensures that all intervention data is maintained, recorded, and summarized, where necessary and reports progress to their Home Supports Coordinator and MSS/Executive Director.
xi) As required is responsible for medication ordering and double checking the medication bubble packages when the order is received.
ii) Maintaining valid First Aid and CPR (Level C) certification and be willing to provide First Aid assistance if necessary.
7. Ensures that all matters relating to the support and care provided to individual(s) served by CPH shall be treated with the utmost confidence and not discussed with people outside the support team of the individual. The above statements reflect the general details considered necessary to describe the principal functions of the job and shall not be construed as a detailed of all work assignments that may be inherent in the job.
This position is considered Safety Sensitive as per the definition set in 2017 by the Canadian Human Rights Commission, whereas if not performed in a safe manner, can cause direct and significant damage to property, and/or injury to the employee, others around them, the public, and/or the immediate environment.
Performance Evaluations and Benefits:
Senior Home Support Leaders are expected to support a minimum of 40 hours in the home and to complete administrative duties per week and allow more hours for on-call availability. Evaluations are completed at 12 months and annually thereafter. Positive Performance evaluations at 12 months result in a role permanency, if available.

Preferred Qualifications:
1. Related post-secondary education in Social Work, Psychiatric Nursing or Nursing
2. Valid Driver’s License
3. Basic computer knowledge and skills including ability to work with Microsoft Office & Outlook.
4. A proven, strong working knowledge and belief in CPH philosophy and Gentle Teaching.
5. A positive understanding of individual rights and Supported Decision Making.
6. A working knowledge of the following: a. Crisis Prevention and Intervention techniques b. Alternative communications
It is desirable that this position sees the importance of the individually oriented support plans for each person served. This position should also demonstrate both creativity and common sense when serving people.
